Once you have reviewed the instructions to proceed to online forms
The following are instructions on how to complete and submit Broken Arrow Public Schools athletic participation forms:

1. The following information will be needed to complete the online forms:

• Student’s ID number
• Student’s full legal name
• Date of birth
• Email address
• Student’s grade in school
• Home phone number and cell phone number
• Student’s current mailing address
• Last school attended with school address
• Primary physician and hospital
• Emergency contacts with phone numbers

2. Student athletes attending the Broken Arrow Freshman Academy will select “Broken Arrow High School” from the pull-down menu under the “School Attending” field on each of the online forms. All required spaces on the forms will need to be completed. If a question is not applicable, please answer with NA or UNKNOWN.

3. To sign the document, click inside the signature box and hold down the mouse button, which allows you to create an “electronic signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an iPad, you may create an electronic signature by holding your finger to the screen or by using a stylus pen.

4. Each form may be printed after all information is completed.

5. When the form is submitted, it will be reviewed by the BAPS athletic trainer and a confirmation email will be sent to the email address that is provided at the bottom of each form.
